Qualifications:
- Plan, allocate, and monitor therapy services in line with the patient’s approved treatment plan. Patients should be observed while receiving treatment, data collected should be gathered and reported to the Physical Therapist (PT).
- Maintain the greatest levels of professionalism while dealing with sensitive or private information. The patient’s best interest should be the first consideration when sharing information with a patient’s family or caregiver.
- Keep accurate and comprehensive records of patient care activities and assessments on the day on which they are performed. Producing patient progress notes for licensed therapists is my job.
- Participate, along with other members of the staff, the patient, and the patient’s family, in the process of care planning and provision, Accurately and in a timely way record and report attendance as well as the provision of services and treatment as well as any other documentation.
- Maintains communication with patients, the Physical Therapist, Clinical Manager(s), and other members of the healthcare team and the community to ensure effective care coordination and referrals to relevant resources.
- Timely document patient information, functional state, treatment plans, progress, status, precautions, etc.
- Assign Performance Improvements. Communicate data gathering and root cause analysis to discover improvement prospects.
- Handle work obligations according to the Company’s Code of Business Conduct, the Corporate Compliance Agreement, and state/federal regulations.
- All of the citizens who are in need of rehabilitative services will get such services thanks to the PTA. This involves providing direct patient care services such as assessing and devising a treatment plan for individuals requiring long term care as well as acute care.
- Recommend to the Physical Therapist feasible modifications to current physical therapy treatment plans and training needed in order to get the best outcomes. Retain and/or submit papers as instructed by the business or location.
Skills:
- a two-year physical therapist assistant degree from an accredited institution Required to have a valid state driver’s license Although it’s a plus, working with the elderly isn’t a must. Typing at a speed of 40 words per minute.
- Effective writing and verbal communication skills, knowledge of suitable therapeutic concepts and practices, knowledge of geriatric therapy principles and practices
- Teaching the patient’s loved ones and other members of the medical team about their role in treatment and how they might help the patient.
- Patients’ responses to therapy and changes in their condition are documented and reported to the supervising therapist, the supervising nurse, and/or the physician.
- Following the plan of care, teaches and assists patients with active and passive exercise, muscle re-education, gait, ADL, transfer, safety, and prosthesis training.
