The Medical Record Clerk is accountable and liable for maintaining health records. The individual is responsible for organizing and maintaining a system of health files and logs pertaining to patient diagnosis, treatment, admissions, discharges, and daily patient census changes. The employee is required to act independently and with good judgment and confidentiality.

  • Assist the Medical Records Supervisor in creating recurring statistical reports and other management information system requirements.
  • Sort and organize loose pages precisely according to defined criteria for paper and electronic scanning.
  • Must be able to use solid judgment beyond a specific set of instructions and apply knowledge to a variety of factual situations.
  • Self-motivated, organized, and able to handle multiple tasks while maintaining strict secrecy are all prerequisites for this position, as is familiarity with the Clinic’s mission and philosophy.
  • Collect and distribute charts and loose sheets from all hospital and/or clinic areas.
  • The Medical Records Clerk must be adaptable to the department’s shifting requirements as well as to any other duties he or she may be given by the Medical Record Director.
  • The medical record clerk will help to clean closed charts so they can be boxed and sent to off-site storage.
  • Each month, it will be the Medical Record Clerk’s job to review the discharge procedure checklist and enter the information into a database.
  • As well as retrieving and generating numbered folders, filing processed medical records, and repairing defects as doctors finish charts, the Medical Record Clerk does a variety of other duties.
  • Must be able to accurately follow established procedures that may require variations and use critical thinking techniques to a variety of practical and emergent scenarios.


  • Working as a medical record technician, medical record clerk, unit secretary or a comparable function in a healthcare setting for at least one year with daily responsibilities that included the processing of computerized medical/health records.
  • Proficient in the use of Microsoft Office programs (Word, Outlook, PowerPoint, and Excel).
  • Knowledge of the fundamentals of standard electronic medical record procedures, methods, and requirements is required to undertake a wide range of routine medical records management.
  • The capacity to spot informational discrepancies and address them appropriately.
  • You must have the ability to work well with others and adapt to a constantly changing work environment.
